The National Cancer Institute of Canada Clinical Trials Group conducted a phase II trial of trimetrexate given in a daily × 5 intravenous bolus schedule every 3 weeks in patients with measurable recurrent anaplastic glioma and limited prior treatment. There were no responses in 14 evaluable patients. We conclude that trimetrexate, given as described, is not an active agent in this disease.

An inquiry into the incidence of cross-infections, complications and return cases in scarlet fever*

A simultaneous investigation into certain epidemiological aspects of scarlet fever was carried out over a 2-year period at Oxford, Cambridge and Cardiff. The results obtained at the three centres are compared and contrasted.
Nose and throat swabs were taken from 648 patients on admission to hospital; group A haemolytic streptococci were demonstrated in the swabs of 75·3 %. Eight days after admission 81·9 % had given positive swabs.
Serological typing showed that 56·9 % of strains were types 1–4. The only other common types were types 11 and 8/25.
The average length of stay in hospital was estimated for all cases, divided into groups according to whether or not they had suffered from complications or been cross-infected. The average for the whole series was 30·6 days; this was increased to 33·4 days for all complicated cases and to 37·6 days for all those cross-infected. It was longest (42·3 days) when there were both complications and cross-infections.
The incidence of cross-infection with fresh serological types of haemolytic streptococci was noted both in hospital and for the first 3 weeks after discharge. In the whole series 20·1 % of cases were cross-infected in hospital. The cross-infection rate was highest at Cardiff (27·9 %) and lowest at Oxford (13·5 %). Of the 123 cross-infected cases, complications occurred in 23·6%.
A total of ninety-eight patients (16·4 %) suffered from complications; in twenty-three of these (23·4%) complications were attributed to cross-infecting strains.
Nose and throat swabs were taken from all patients shortly before discharge; 60·3% were still carrying haemolytic streptococci.
Data were obtained regarding the rate at which cases became free of haemolytic streptococci. By the 10th week of the disease only 3·2 % of those who had not been cross-infected were carrying streptococci. Cross-infected patients were slower in becoming negative; at the 10th week 25·8% were still carriers.
There were fourteen return cases (return case rate 2·1%) following the return home of eleven primary cases (infecting case rate 1·7 %).
Swabbing of home contacts at the time of the patient's admission to hospital showed that one or more contacts of 25·5% of cases carried in their naso-pharynx the same type of haemolytic streptococci as that infecting the patient; 12·8% of the contacts swabbed were positive. During the first 3 weeks after discharge 24·6 % of contacts, that is one or more contacts of 55·5% of cases, carried a type that had been harboured by the patient while in hospital.
It was noted that in a particular area the common types that gave rise to scarlet fever also commonly caused other streptococcal infections of the upper respiratory tract. Type 12, however, though frequently found in other streptococcal infections and in healthy carriers, appeared only rarely to cause scarlet fever.
It is suggested that the low incidence of complications, less intimate relationship between complications and cross-infections and low return case rate found in this investigation, as compared to earlier observations made in England, are associated with lowered virulence and invasive powers of the strains of Str. pyogenes responsible for the much milder scarlet fever now prevalent.
The bacteriological findings confirm the wisdom of modern fever hospital practice of early discharge from hospital. It is suggested that discharge within 3 weeks should be the universally adopted rule.
Risk of Hand or Glove Contamination After Contact With Patients Colonized With Vancomycin-Resistant Enterococcus or the Colonized Patients' Environment

Objective.
To estimate the level of hand or glove contamination with vancomycin-resistant enterococci (VRE) among healthcare workers (HCWs) who touch a patient colonized with VRE and/or the colonized patient's environment during routine care.
Design.Structured observational study.
Setting.Medical intensive care unit of a 700-bed, tertiary-care teaching hospital.
Participants.VRE-colonized patients and their caregivers.
Methods.We obtained samples from sites on the intact skin of 22 patients colonized with VRE and samples from sites in the patients' rooms, before and after routine care, during 27 monitoring episodes. A total of 98 unique HCWs were observed during 131 HCW observations. Observers recorded the sites touched by HCWs. Culture samples were obtained from HCWs' hands and gloves before and after care.
Results.VRE were isolated from a mean (±SD) of 55% ± 24% of patient sites (n = 256) and 17% ± 12% of environmental sites (n = 1,572). Most HCWs (131 [56%]) touched both the patient and the patient's environment; no HCW touched only the patient. Of 103 HCWs whose hand samples were negative for VRE when they entered the room, 52% contaminated their hands or gloves after touching the environment, and 70% contaminated their hands or gloves after touching the patient and the environment (P = .101). In a univariate logistic regression model, the risk of hand or glove contamination was associated with the number of contacts made (odds ratio, 1.1 [95% confidence interval, 1.01-1.19). In a multivariate model, the effect of the number of contacts could not be distinguished from the effect of type of contact (ie, touching the environment alone or touching both the patient and the environment). Overall, 37% of HCWs who did not wear gloves contaminated their hands, and 5% of HCWs who wore gloves did so (an 86% difference).
Conclusion.HCWs were nearly as likely to have contaminated their hands or gloves after touching the environment in a room occupied by a patient colonized by VRE as after touching the colonized patient and the patient's environment. Gloves were highly protective with respect to hand contamination.
Dietary patterns, insulin sensitivity and adiposity in the multi-ethnic Insulin Resistance Atherosclerosis Study population

Epidemiological investigations increasingly employ dietary-pattern techniques to fully integrate dietary data. The present study evaluated the relationship of dietary patterns identified by cluster analysis with measures of insulin sensitivity (SI) and adiposity in the multi-ethnic, multi-centre Insulin Resistance Atherosclerosis Study (IRAS, 1992–94). Cross-sectional data from 980 middle-aged adults, of whom 67% had normal and 33% had impaired glucose tolerance, were analysed. Usual dietary intake was obtained by an interviewer-administered, validated food-frequency questionnaire. Outcomes included SI, fasting insulin (FI), BMI and waist circumference. The relationship of dietary patterns to log(SI+1), log(FI), BMI and waist circumference was modelled with multivariable linear regressions. Cluster analysis identified six distinct diet patterns – ‘dark bread’, ‘wine’, ‘fruits’, ‘low-frequency eaters’, ‘fries’ and ‘white bread’. The ‘white bread’ and the ‘fries’ patterns over-represented the Hispanic IRAS population predominantly from two centres, while the ‘wine’ and ‘dark bread’ groups were dominated by non-Hispanic whites. The dietary patterns were associated significantly with each of the outcomes first at the crude, clinical level (P<0·001). Furthermore, they were significantly associated with FI, BMI and waist circumference independent of age, sex, race or ethnicity, clinic, family history of diabetes, smoking and activity (P<0.004), whereas significance was lost for SI. Studying the total dietary behaviour via a pattern approach allowed us to focus both on the qualitative and quantitative dimensions of diet. The present study identified highly consistent associations of distinct dietary patterns with measures of insulin resistance and adiposity, which are risk factors for diabetes and heart disease.

Summary
Introduction
Idiopathic hemochromatosis is a familial genetic disorder in which iron absorption consistently exceeds losses of body iron. This leads to accumulation of iron in multiple body organs.When iron concentrations become sufficiently excessive, cellular damage and destruction occur with organ dysfunction and failure. This may become manifest as either cirrhosis of the liver, hepatocellular carcinoma, diabetes mellitus, cardiomyopathy, hypogonadism, or arthritis, or a combination of these disorders. Because hemochromatosis usually becomes clinically manifest during the fifth and sixth decades of life, it usually takes many years of iron accumulation before a homozygote seeks medical attention. Presumably, this long delay occurs because body organs possess a capability to store iron far in excess of body needs. Once the iron-binding capacity of tissues are exceeded, ionic iron becomes available to produce ‘free radical’ formation and cellular damage. An enigma that remains unexplained is that studies of the prevalence of homozygous hemochromatosis in Caucasians (∼0.6%) indicate that genotypic hemochromatosis is at least ten times more commonplace than clinically manifested phenotypic hemochromatosis (1 in 20000 hospitalizations). This suggests that factors in addition to a single genetic mutation are necessary to produce massive iron overloading with organ damage.
Although the genetic defect for about 85% of Caucasian hemochromatotics was recently identified (HLA-H, C282Y), the method by which it enhances iron accumulation by the body remains unknown. One possibility is that the genetic abnormality acts directly upon the intestinal absorptive cell to enhance the absorption of iron.
